A PRI (Patient Review Instrument) is an assessment tool that takes an overall picture of your loved ones cognitive, physical and medical condition. This helps to determine their level and type of care needed.
When your loved one is no longer able to care for themselves safely at home New York State requires that a PRI be performed for anyone entering a skilled nursing facility.

For family members entering a NYS nursing home
Step 1: Arrange for a PRI Assessment
Arrange for a PRI Assessment. A PRI is required by law and is valid for 90 days. A PRI determines the level of care and type of facility needed. A nurse will call with a time and location that works with your schedule.
Step 2: Medical Summary
Families can get a Medical history/summary from their primary care physician (PCP).
Step 3: Finances
Be aware of your loved ones finances. Apply for Medicaid if necessary.
Step 4: Nursing Home Preferences
Together with your family make a list of potential nursing home options based on services and specialized care that each home has to offer.
Step 5: Apply to Nursing Homes
Contact nursing home admissions offices. We recommend inquiring to multiple nursing facilities.
Step 6: Follow Up
Apply through the nursing facilities admissions office with your completed PRI assessment. Frequent follow up may be necessary.
